RESUMO

How pain and sensorimotor behavior interact has been the subject of research and debate for many decades. This article reviews theories bearing on pain-sensorimotor interactions and considers their strengths and limitations in the light of findings from experimental and clinical studies of pain-sensorimotor interactions in the spinal and craniofacial sensorimotor systems. A strength of recent theories is that they have incorporated concepts and features missing from earlier theories to account for the role of the sensory-discriminative, motivational-affective, and cognitive-evaluative dimensions of pain in pain-sensorimotor interactions. Findings acquired since the formulation of these recent theories indicate that additional features need to be considered to provide a more comprehensive conceptualization of pain-sensorimotor interactions. These features include biopsychosocial influences that range from biological factors such as genetics and epigenetics to psychological factors and social factors encompassing environmental and cultural influences. Also needing consideration is a mechanistic framework that includes other biological factors reflecting nociceptive processes and glioplastic and neuroplastic changes in sensorimotor and related brain and spinal cord circuits in acute or chronic pain conditions. The literature reviewed and the limitations of previous theories bearing on pain-sensorimotor interactions have led us to provide new perspectives on these interactions, and this has prompted our development of a new concept, the Theory of Pain-Sensorimotor Interactions (TOPSMI) that we suggest gives a more comprehensive framework to consider the interactions and their complexity. This theory states that pain is associated with plastic changes in the central nervous system (CNS) that lead to an activation pattern of motor units that contributes to the individual's adaptive sensorimotor behavior. This activation pattern takes account of the biological, psychological, and social influences on the musculoskeletal tissues involved in sensorimotor behavior and on the plastic changes and the experience of pain in that individual. The pattern is normally optimized in terms of biomechanical advantage and metabolic cost related to the features of the individual's musculoskeletal tissues and aims to minimize pain and any associated sensorimotor changes, and thereby maintain homeostasis. However, adverse biopsychosocial factors and their interactions may result in plastic CNS changes leading to less optimal, even maladaptive, sensorimotor changes producing motor unit activation patterns associated with the development of further pain. This more comprehensive theory points towards customized treatment strategies, in line with the management approaches to pain proposed in the biopsychosocial model of pain.

RESUMO

Objectives: The current study aims to investigate the protective effect of iron oxide nanoparticles capped with curcumin (FeONPs-Cur) against motor impairment and neurochemical changes in a rat model of Parkinson's disease (PD) induced by reserpine. Materials and Methods: Rats were grouped into control, PD model induced by reserpine, and PD model treated with FeONPs-Cur (8 rats/group). The open field test was used to assess motor activity. The concentration of dopamine (DA), norepinephrine (NE), serotonin (5-HT), lipid peroxidation (MDA), reduced glutathione (GSH), and nitric oxide (NO), and the activities of Na+,K+,ATPase, acetylcholinesterase (AchE), and monoamine oxidase (MAO) were determined in the midbrain and striatum. Data were analyzed by ANOVA at P-value<0.05. Results: The PD model exhibited a decrease in motor activity. In the midbrain and striatum of the PD model, DA, NE, and 5-HT levels decreased significantly (P-value<0.05). However, an increase in MAO, NO, and MDA was observed. GSH, AchE and Na+,K+,ATPase decreased significantly in the two brain areas. FeONPs-Cur prevented the decline of dopamine and norepinephrine and reduced oxidative stress in both areas. It also prevented the increased MAO activity in the two areas and the inhibited activity of AchE and Na+,K+,ATPase in the midbrain. These changes were associated with improvements in motor activity. Conclusion: The present data indicate that FeONPs-Cur could prevent the motor deficits induced in the PD rat model by restoring dopamine and norepinephrine in the midbrain and striatum. The antioxidant activity of FeONPs-Cur contributed to its protective effect. These effects nominate FeONPs-Cur as an antiparkinsonian candidate.

RESUMO

BACKGROUND: This study aimed to verify leisure-time physical activity trends over 15 years and monitor inequalities according to gender, self-reported skin color, and socioeconomic position in a Southern Brazilian city. A secondary aim is to evaluate intersectionalities in physical activity. METHODS: Trend analysis using 3 population-based surveys carried out in 2004, 2010, and 2021. Main outcome assessed was the prevalence of physical activity according to recommendations (150 min/wk). Inequalities dimensions measured were sex, self-reported skin color, and wealth. Intersectionalities were evaluated using Jeopardy index combining all inequality dimensions. Trend analysis was performed using least-squares weighted regression. RESULTS: We included data from 3090, 2656, and 5696 adults in 2004, 2010, and 2021, respectively. Prevalence of physical activity remains stable around 25% in the 3 years. In the 3 periods evaluated, men presented a prevalence in average 10 percentage points higher than women (SII2004 = -11.1 [95% confidence interval, CI, -14.4 to -7.8], SII2021 = -10.7 [95% CI, -13.7 to -7.7]). Skin color inequalities did not present a clear pattern. Richest individuals, in general presented a prevalence of leisure-time physical activity level 20pp higher than poorest ones (SII2004 = 20.5 [95% CI, 13.7 to 27.4]; SII2021 = 16.7 [95% CI, 11.3 to 22.0]). Inequalities were widely marked, comparing the most privileged group (represented by men, the wealthiest, and White) and the most socially vulnerable group (represented by women, the poorest, and Black/Brown). The Slope Index of Inequality for intersectionalities was -24.5 (95% CI, -31.1 to -17.9) in 2004 and -18.8 in 2021 (95% CI, -24.2 to -13.4). CONCLUSIONS: Our analysis shows that women, Black/Brown, and poor present lower leisure-time physical activity level. This group is often neglected regarding other health and social outcomes.

RESUMO

Following the European Commission Endocrine Disruptor Criteria, substances shall be considered as having endocrine disrupting properties if they (a) elicit adverse effects, (b) have endocrine activity, and (c) the two are linked by an endocrine mode-of-action (MoA) unless the MoA is not relevant for humans. A comprehensive, structured approach to assess whether substances meet the Endocrine Disruptor Criteria for the thyroid modality (EDC-T) is currently unavailable. Here, the European Centre for Ecotoxicology and Toxicology of Chemicals Thyroxine Task Force and CropLife Europe propose a Thyroid Function-Related Neurodevelopmental Toxicity Testing and Assessment Scheme (Thyroid-NDT-TAS). In Tier 0, before entering the Thyroid-NDT-TAS, all available in vivo, in vitro and in silico data are submitted to weight-of-evidence (WoE) evaluations to determine whether the substance of interest poses a concern for thyroid disruption. If so, Tier 1 of the Thyroid-NDT-TAS includes an initial MoA and human relevance assessment (structured by the key events of possibly relevant adverse outcome pathways) and the generation of supportive in vitro/in silico data, if relevant. Only if Tier 1 is inconclusive, Tier 2 involves higher-tier testing to generate further thyroid- and/or neurodevelopment-related data. Tier 3 includes the final MoA and human relevance assessment and an overarching WoE evaluation to draw a conclusion on whether, or not, the substance meets the EDC-T. The Thyroid-NDT-TAS is based on the state-of-the-science, and it has been developed to minimise animal testing. To make human safety assessments more accurate, it is recommended to apply the Thyroid-NDT-TAS during future regulatory assessments.

RESUMO

Spontaneously hypertensive rats (SHRs) are widely accepted for modeling essential hypertension and Attention deficit hyperactivity disorder (ADHD). However, data concerning central nervous system changes associated with behavioral responses of this strain and usage of Wistar Kyoto (WKY) rats as controls are confounding. The objective of the present study was to assess the impact of anxiety and motor activity on the cognitive responses of SHRs compared to Wistar and WKY rats. In addition, the role of brain-derived neurotrophic factor (BDNF) in the hippocampus on cognitive behavior and seizure susceptibility in the three strains was evaluated. In Experiment#1, SHR demonstrated impulsive responses in the novelty suppression feeding test accompanied by impaired spatial working and associative memory in the Y maze and object recognition test compared with the Wistar rat but not WKY rats. In addition, the WKY rats exhibited diminished activity compared to Wistar rats in an actimeter. In Experiment#2, the seizure susceptibility was assessed by 3-min electroencephalographic (EEG) recording after two consecutive injections of pentylenetetrazol (PTZ) (20+40 mg/kg). The WKY rats were more vulnerable to rhythmic metrazol activity (RMA) than the Wistar rats. In contrast, Wistar rats were more prone to generalized tonic-clonic seizures (GTCS) than WKY rats and SHRs. Control SHR had lower BDNF expression in the hippocampus compared to Wistar rats. However, while the BDNF levels were elevated in the Wistar and WKY rats after PTZ injection, no change in this signaling molecule was observed in the SHR in the seizure condition. The results suggest Wistar rats as a more appropriate control of SHR than WKY rats for studying memory responses mediated by BDNF in the hippocampus. The higher vulnerability to seizures in Wistar and WKY rats compared to SHR might be linked to PTZ-induced decreased expression of BDNF in the hippocampus.

Public open spaces (POSs) are means to ensure one's right to recreation and health. The objective of this essay is to present reflections and evidence on how these spaces contribute to promoting physi-cal activity (PA). Understanding how to access (proximity/distance), quantity and diversity, and the surrounding and internal conditions of places (quality, structure, aesthetics, safety) can affect the use of POSs is imperative for public management actions. Thinking of POS use beyond PA by recog-nizing its social, economic, and cultural benefits can be fundamental to reduce inequalities regarding access to these places. POS-related actions and demands are intersectoral, multiprofessional, and interdisciplinary, thus requiring political, academic, and community involvement and commitment for PA promotion

Aging promote changes in the autonomy and dependence of the elderly, which associated with adverse factors can lead the individual to a state of fragility. However, the practice of physical exercises has shown positive effects on functionality that can contribute in a reduction in the risk of vulnerability of the frail older adults. Objective: The aim of the study was to evaluate the effect of exercise in the domains assessed by the Clinical-Functional Vulnerability Index (IVCF-20) in frail older adults. Method: Fifty-six frail older adult's people of both sexes, attended at the Health Care Reference Center for the Older Adults - Eny Faria De Oliveira - CRASI, were assessed using the IVCF-20 before and after 10 physiotherapy sessions. The 8 domains that make up the IVCF-20 were analyzed individually. Friedman's ANOVA test was used for pre- and post-rehabilitation comparisons intragroup. Results: Significant differences were observed in the domains of activities of daily living, cognition, mood, mobility, and post-training communication (p<0.01) in the female and male group. Conclusion: exercise programs can contribute to improving the mobility, cognition, and functionality of frail older adults.

RESUMO

Health promotion programs can reduce morbidity and mortality from chronic diseases, as well as public spending on health. The current study aims to evaluate the effects of the Health Gym Program on expenditures on hospitalizations for stroke in the state of Pernambuco, Brazil. This public policy impact assessment used an econometric model that combines the difference-in-difference estimator with propensity score matching. Data referring to the health, demographic, and socioeconomic characteristics of the 185 municipalities in Pernambuco were collected for the period from 2007 to 2019. Validation tests were carried out of the empirical strategy, the estimation of models with fixed effects for multiple periods and validation post-tests, and robustness of the results. In total, US$ 52,141,798.71 was spent on hospitalizations for stroke, corresponding to 4.42% of the expenses on hospitalizations for all causes over the period studied. Municipalities that implemented the Health Gym Program spent 17.85% less on hospitalizations for stroke than municipalities that did not. The findings of this study indicate that the Health Gym Program was effective in reducing expenses with hospitalizations for stroke and that its implementation has the potential to reduce expenses related to rehabilitation, sick leave, and early retirement.

RESUMO

BACKGROUND: Most research on patterns of motor activity has been conducted on adults with mood disorders, but few studies have investigated comorbid attention-deficit/hyperactivity disorder (ADHD) or temperamental factors that may influence the clinical course and symptoms. Cyclothymic temperament (CT) is particularly associated with functional impairment. Clinical features define both disorders, but objective, biological markers for these disorders could give important insights with regard to pathophysiology and classification. METHODS: Seventy-six patients, requiring diagnostic evaluation of ADHD, mood or anxiety disorders were recruited. A comprehensive diagnostic evaluation, including the CT scale of the Temperament Evaluation of Memphis, Pisa, Paris and San Diego - Auto-questionnaire (TEMPS-A), neuropsychological tests and actigraphy, was performed. ADHD was diagnosed according to the DSM-IV criteria. There was a range of different conditions in this clinical sample, but here we report on the presence of CT and ADHD in relation to motor activity. Twenty-nine healthy controls were recruited. We analyzed motor activity time series using linear and nonlinear mathematical methods, with a special focus on active and inactive periods in the actigraphic recordings. RESULTS: Forty patients fulfilled the criteria for ADHD, with the remainder receiving other psychiatric diagnoses (clinical controls). Forty-two patients fulfilled the criteria for CT. Twenty-two patients fulfilled the criteria for ADHD and CT, 18 patients met the criteria for ADHD without CT, and 15 patients had neither. The ratio duration of active/inactive periods was significantly lower in patients with CT than in patients without CT, in both the total sample, and in the ADHD subsample. CONCLUSIONS: CT is associated with objectively assessed changes in motor activity, implying that the systems regulating motor behavior in these patients are different from both healthy controls and clinical controls without CT. Findings suggest that actigraphy may supplement clinical assessments of CT and ADHD, and may provide an objective marker for CT.

RESUMO

OBJECTIVES: This study was conducted to develop the Mobility to Participation Assessment Scale for Stroke (MPASS) and assess its content validity, internal consistency, inter-rater and intra-rater reliability, and convergent validity in people with stroke living in the community. METHODS: The MPASS was developed using published data on mobility-related activity and participation timing in elderly individuals, and then reviewed by community physical therapists. Content validity was established by reaching a consensus of experienced physical therapists in a focus group. The MPASS was scored for 32 participants with stroke (mean age 61.75±4.92 years) by 3 individual testers. Reliability was examined using the intraclass correlation coefficient (ICC), internal consistency using the Cronbach alpha coefficient (α), and convergent validity using the Pearson correlation coefficient (r) to compare the MPASS to the Modified Rivermead Mobility Index as a referent test of mobility. RESULTS: The MPASS consists of 8 items, and its scoring system provides information on the ability of people with stroke to reach a movement level enabling them to live in society, including interactions with other people and safe living in the community. The inter-rater and intra-rater reliability were excellent (ICC, 0.948; 95% confidence interval [CI], 0.893 to 0.982 and ICC, 0.967; 95% CI, 0.933 to 0.989, respectively). Internal consistency was good (α=0.877). The convergent validity was moderate (r=0.646; p<0.001). CONCLUSIONS: The newly developed MPASS showed acceptable construct validity and high reliability. The MPASS is suitable for use in people with stroke, especially those who have been discharged and live in the community with the ability to initiate sitting.

Abstract The scope of this study was to analyze the organizational structure, financing and supply of programs and actions of bodily practices and physical activities (PCAF) considering the tripartite management of the Unified Health System (SUS). Focusing on the years2018 to 2020, information was requested from the Ministry of Health (MS), the 26 state health secretariats, the Federal District (SES) and the 26 municipal health secretariats in the capitals (SMS). 40 responses were received (rate of 74%). Based on the relevance of the Expanded Center for Family Health and Primary Care (Nasf AB) in the provision of PCAF, research was also carried out in information systems. It was observed that only the MS, two SES' and four SMS' had a specific PCAF management structure; the possibility of financing was identified by the MS, in six SES' and eight SMS'. With respect to the programs, they seem to be limited to the Health Academy. In relation to the offer of PCAF, it was observed that from 2015 to 2020 the percentage of participation of Nasf AB was always higher than 70%, reaching 82.3%. Thus, the panorama revealed in the MS, SES and SMS capitals sets up a timid arrangement of essential elements of organizational structure, funding, programs, and actions for the PCAF to advance as a public health policy.

Abstract: Objective: To analyze the effect of Physical Activity (PA) on the Quality of Life (QL) of people aged 15 years and over considering sociodemographic, epidemiological, and psychosocial characteristics collected in a national survey carried out in Chile. Materials and methods: Quantitative-analytical-transversal developed in Chile 2020. Descriptive and inferential statistics. Results: Average age of 49 years, 63% female. 15% reported practicing some sport or training intensely; 75% have a good or very good QL, and 53% have a good or very good QL in health. Those who practiced an intense sport were more likely to have a good or very good QL (OR= 1.58; 95%CI= 1.28,1.96) and QL associated with health (OR= 1.67; 95%CI= 1.40,1.99) compared to those who did not do it. Conclusions: PA is an aspect that impacts people's QL, cheap, easy to establish, and positively impacts the general health of the population.

RESUMO

INTRODUCTION: This study aims to compare differences regarding physical activity (PA) indicators and TV viewing according to location of residence and sociodemographic variables in Brazil. METHODS: This cross-sectional study with individuals aged 18 years or older using data from the Brazilian National Health Survey was carried out in 2013. PA (total and domains) and TV viewing (≥3 hours) were measured by self-report. Stratifiers were location of residence, sex, age, wealth quintiles and regions of the country. RESULTS: This study included 60 202 individuals (49 245 from urban areas). In general, total PA was not different according to location of residence (urban 59.4%; rural 57.4%). Higher differences were found for leisure-time PA (urban 24.1%; rural 13.9%), work-based PA (urban 12.9%; rural 21.1%) and TV viewing of more than 3 hours (urban 30.1%; rural 21.4%). The differences between urban and rural areas were enhanced according to sex, age and wealth quintiles. CONCLUSION: The differences between urban and rural areas vary according to PA domains and population groups. To explore specific domains of PA in each location of residence may improve the understanding of the analyzed outcomes in different communities and consequently guide future interventions.

The objective was to verify how the race/color variable has been stratified in Brazilian studies that had physical activity (PA) as a dependent variable and the association between race/color with leisure-time physical activity (LTPA). In the 20 studies found, nine forms of stratification were observed, the most used being: white, brown, black. The "white" category was included in all studies. Twelve studies analyzed the relationship of race/color with LTPA and five of these observed some association, varying according to age group. Conclude that race/color was investigated in different ways, that less than half of the studies observed some relationship with LTPA and that in studies that observed an association, the direction of this was different depending on the age group.

BACKGROUND: This study aims to investigate inequalities in leisure-time physical activity (PA) practice amid the COVID-19 pandemic in a southern Brazilian city. METHODS: Four repeated population-based surveys were carried out. PA was collected using a questionnaire proposed by the authors and an adapted version of the leisure-time section of the International Physical Activity Questionnaire. The sociodemographic variables were sex, age, skin color, and education level. The participants also answered questions regarding social distancing measures compliance. Descriptive analyses based on proportions and their respective P values for categorical variables were presented. The chi-square test for heterogeneity and linear trend was used. RESULTS: The sample is composed of 1556 individuals (66.1% female). Overall, between rounds 1 and 2, PA prevalence declined, followed by a gradual increase thereafter. PA practice during the pandemic was higher among men, individuals with higher education level, and individuals with white skin color in all rounds. In rounds 2, 3, and 4, PA was lower among individuals who were practicing more social distancing. CONCLUSION: To tackle the PA inequalities, policymakers and stakeholders need to confront disparities, defending greater availability of public policies that are attentive to inequalities, especially regarding gender, skin color, and educational level, to promote PA as a human right.

RESUMO

BACKGROUND: To analyze the relationship between different physical activity (PA) domains and adiposity indicators in breast cancer survivors. METHODS: This is a cross-sectional study, composed of 115 breast cancer (BC) survivors. Measurements of weight, height, waist circumference (WC), and hip circumference were objectively collected, as well as the analysis of body fat percentage through bioelectrical impedance analysis. The PA level was assessed by the Baecke questionnaire including domains related to occupational activity, leisure-time activity, and transportation, while the time spent in sedentary behavior was assessed through self-reported screen-time. Socioeconomic status was assessed through the Brazilian criteria for economic classification and considered as a covariate. RESULTS: BC survivors with higher PA levels in leisure-time activity/sport presented reductions in body fat percentage (ß = 1.13%; CI = -2.06; -0.21), BMI (ß = 0.80 kg/m²; CI = -1.50; -0.11), WC (ß = 1.89 cm; CI = -3.54; -0.23), and waist-to-height ratio (ß = 0.13 cm; CI = -0.23; -0.02). Similar values were observed in PA during transportation and in total. There was no correlation between PA at work and adiposity indicators. CONCLUSION: Different PA domains were inversely related to body adiposity in BC survivors.
